Immunization Requirements for 2020-2021

The Liverpool Central School District, pursuant to Section 2164 of the Public Health Law, requires a statement from a physician or clinic or previous health record which verifies immunization for the following (as of September 2020):

  • Diphtheria and Tetanus toxoid-containing vaccine and Pertussis vaccine (DTaP/DTP/Tdap/Td)
  • Tetanus and Diphtheria toxoid-containing vaccine and Pertussis vaccine booster (Tdap)
  • Polio vaccine (IPV/OPV)
  • Measles, Mumps and Rubella vaccine (MMR)
  • Hepatitis B vaccine Varicella (Chickenpox) vaccine
  • Meningococcal (MenACWY)

For the 2020-2021 school year, all students must receive immunizations as required by New York State Health Law, unless the student’s parent/guardian can obtain a certification from a treating physician that the required immunization(s) would be detrimental to the child’s health. This includes any LCSD student participating in remote learning only for the first semester of the 2020-2021 school year. Children who are enrolling in gradeless classes should meet the immunization requirements of the grades for which they are age equivalent.

Required immunizations must be received no later than 14 days after the first day of school (for the Liverpool Central School District that day is Monday, September 14, 2020). Within 30 days of the first day of school, a parent/guardian must show that appointments have been made for the student to receive all required follow-up doses of the immunizations. A student will be excluded from school without the required certificate/appointments.
